Our therapists are familiar with working in mental health and disability settings. We understand NDIS legislation, including the importance of good evidence to substantiate your needs within an NDIS Plan review meeting.

We can assist you to work towards the aspirations you have for your life.

Families can feel comfortable that our approach is family inclusive. Wherever possible, and where it is appropriate, we consider and welcome the family in our discussions and consider your lived expertise together with the person you support.

While not exhaustive, these are some of the services we may provide:

  • Create skills development materials to guide your care team, based on your preferences.

  • Creation of documentation outlining important considerations for your health and support management, to minimise the times you may need to repeat your story, and your needs.

  • Suggestions and recommendations for additional NDIS supports. Provide guidance within NDIS processes.

  • Education about your health situation and how it affects all of you - wherever possible as a shared conversation including the perspective of the person with a lived experience at the centre.

  • Exploration together to manage difficult feelings that arise from past trauma or personal experiences that limit self-belief and progress toward your personal aspirations.

  • Holding a safe space with you in anxious or difficult days. Supporting you to see your strengths, and identify together all you have already achieved.

  • Support to consider ways of improving life through NDIS supports at difficult times - strategies, recommendations, skilled listening. Encouragement and reassurance.

Care coordination

Some families engage with multiple service systems in support of their family member with significant and often co-occurring needs.

Our therapists are skilled in working in complex team situations. Even if you don’t have a Specialist Support Coordinator, we can help.

Services that Human Approach may provide include:

  • Design a system for daily practical support. This can incorporate training to assemble and/or communicate with and manage disability and mental health teams.

  • Clarify roles of team members (professional, clinical, NDIS, informal supports (family)

  • Assist in NDIS Access requests, plan reviews, or Act as an independent advocate (fee for service only)

With our expertise in working with systems, you can improve your experience within health, judicial and community support services.

We work in a multidisciplinary team approach with your Occupational Therapist, Speech Therapist, clinical services, and other community supports important to you.

Housing

Contact us to assist with report writing and evidencing the needs of yourself or your family member toward an effective Supported Independent Living (SIL) or Supported Disability Accommodation (SDA) application.

Some of our team have engaged in the UpSkilled training from the Summer Foundation, and regularly connect with local networks to provide quality assistance in housing support. We can put you in touch with the right person to assist you.
