The role of NDIS support coordinators
Some of the support coordination work carried out regularly by the Human Approach team includes working together with you to:
Understand your NDIS plan and set up a clear budget for your supports
Listen effectively, at times of crisis and making time to hear your experience along the way
Consider, and access, support services you need and prefer
Maintain regular communication with your professionals and service providers
Ensure you are happy with your support service providers – and helping to resolve issues that may arise
Prepare and coordinate collation of reports for the National Disability Insurance Agency (NDIA)
Support you in NDIS reviews with the NDIA

Human Approach supports NDIS participants in Melbourne and beyond with a Plan Managed or Self Managed NDIS Plan. We assist regional and interstate clients.
Individuals with mental ill-health or an experience of invisible disability often find that their needs fall in the gaps in services. There just isn’t much available that might suit their needs. At Human Approach we want to provide you clarity about services that are a fit for you, to ease the way. We will be honest about the options, and work together with you.
We assist those with invisible disability and chronic ill-health, such as intellectual disability, Autism, and dual disability - where Autism or intellectual disability and mental ill-health occurs together.
Where several family members have support needs, we can assist to improve the flow of supports to all of you within your home. We support people with long histories of engagement with hospitals and other health services.
Families often share with us that they find the coordination of support within their NDIS Plan time consuming and stressful. Our clients appreciate our skills in efficiently locating services and navigating NDIS processes such as reviews, and gathering documentation.
Therapist-led NDIS support coordination helps clients make sense of the system, determine exactly what their needs are, access services and have solutions implemented that fit in to their life. We will listen to you and work together to make sense of how to create a lifestyle you prefer, with supports that understand and work with you.

designing a system for daily practical support
clarifying the roles of professionals in your team
encouraging effective communication
to make the most of your NDIS funding and have a better experience each day.
We may also be able to assist you with:
Finding appropriate accommodation, or to submit a housing report
Helping you to receive the National Disability Insurance Scheme (NDIS) funding appropriate to your needs, if you are considering applying
Ensuring you fully understand your rights
Helping you to lodge complaints
Helping you to engage an advocate if required, or representing as an advocate in certain circumstances (fee for service)
Supporting you maintain control over your decisions in everyday life
